Thank you for purchasing a 12 Survivors product! Please fill out the form and submit it. Please only submit once, as sometimes the confirmation screen does not appear, but only a blank box. After submission, each RA request is processed manually during normal business hours to ensure accuracy and quality of service. Please allow up to three business days for a response from our Returns Department. We will email you a Return Authorization Number (RA#) and all the info you will need to get the unit returned. Be aware that sometimes e-mails with attachments from addresses not in your contact list may be forwarded to your spam box without your knowledge. Once your information is processed your case will be assigned an Return Authorization Number that will be used to track your return upon arrival to our facility and ensure replacement or repair.
